Living Well is a FREE, voluntary program designed to help people living with chronic disease, including diabetes, heart failure and chronic obstructive lung disease (also known as, COPD), better understand the facts and feelings surrounding their condition. A Community Wellness Coach, a nurse who will work together with their health care provider to help them take control of their disease, staffs this program. The Coach also works with a Registered Dietician and Diabetic Educator to help teach people how to shop for and prepare food that will help them better manage their diabetes. |

PATH (Personal Action Toward Health) Classes
 This is a Stanford research based program that is being offered through out Michigan- with many options in northwest Michigan.
People with long term health concerns are learning to manage their health and live life to its fullest because they took the PATH!
