Eleven County Service Area
The NMDI service area consists of the following eleven counties: Antrim, Otsego, Crawford, Kakaska, Roscommon, Missaukee, Wexford, Grand Traverse, Benzie, Manistee and Leelanau.
Our HistoryThe Northern Michigan Diabetes Initiative began in 2006 as a collaborative effort between Munson Healthcare, Priority Health, Northern Michigan's Diabetes Outreach Network (TIPDON) and stakeholders from the identified Munson Healthcare geographic service area that includes eleven counties( see the map above). The Steering Committee consists of a representative from Munson Medical Center, Priority Health, TIPDON (Tip of the Mit Diabetes Outreach Network), local physicians, diabetes educators and public health representation.
The Steering Committee made presentations to key regional partners on the Munson Healthcare Community Health Board advocating that diabetes be identified as a critical health care problem in the 11 county region. Resources from all three agencies formed the basis of the proposal for Munson and TIPDON's Strategic Plan forming the basis of the Initiative's plan. The funders of the NMDI have committed to providing ongoing support. Providing education and awareness is the primary role of the NMDI. A Community Diabetes survey representative of the 11 county-service area provides data that is being used to springboard educational efforts. Initially primary care physicians are receiving tool kits and educational in services on diabetes quality standards of care. Public education occurs through mailings, media representation, focus groups and public seminars.
Stakeholder meetings occur multi-annually and promote involvement and direction from the 11 county service area. This input is channeled through the steering committee, the NMDI objectives and small work groups which are collaborated and guided by the NMDI coordinator and the overriding vision- A reduction in the incidence of diabetes and improvement in the care of people with diabetes.
